Swimming Lessons at White Oak from Wednesday 10th September onwards

I agree to my child's participation in swimming lessons as per the details in the email I received. I agree with my child's participation in the swimming lessons and acknowledge the need for them to behave responsibly.

This form is valid for the said school lessons from the date you complete it, for the period of time your child attends the lessons. The onset will automatically expire after this time. We hold your records securely until the lessons have ended and all children have returned to school, after which they are retained securely for 1 month in case of any queries, then safely destroyed.
